Our computer-fillable referral forms are
available at www.discradiology.nycc.edu. Fill
out the referral form completely and print it.
Mail the imaging study with the referral form
enclosed. Hard-copy films and DICOM CDs
are both acceptable. Your films will be mailed
back the day after receipt.
For Teleradiology Services:
DICOM files
can be sent electronically to our PACS. Contact
